- Last two pay stubs and most recent tax return for all borrowers named on the mortgage
- Proof of income, such as salary, wages, child support, spousal support, Social Security, pension and any other income
- Bank account statements (last two months)
- List of major monthly bills, including mortgage payments, child care expenses, medical and dental expenses, food, clothing, utilities, educational expenses, credit card and other loan payments, car payments, car insurance, and other expenses
- Signed and completed Personal Budget Worksheet
